Its easy to change from the DD Waiver to the Mi Via Waiver

  • Contact Your DDSD Regional Office (see map above) to request a Mi Via Transfer Form

  • Select a Consultant

  • Select a Vendor

  • Complete the paperwork with your Consultant

  • Start with Mi Via

​

 

Please Remember: If your client has significant medical needs, the Mi Via Waiver may not be a good fit.
